EJACULATION

  • Consists in the expulsion of semen through the urethral meatus. (¹,²)
  • Normal ejaculatory volume ranges between 2-7 mL and is made up of sperm and products from adjacent glands (urethral, ​​bulbourethral, ​​prostate, seminal vesicles, epididymis, and testicles). (³)
  • Less than 10% of the volume corresponds to sperm and more than 90% to seminal fluid (fructose, amino acids, electrolytes). (³)
  • Varies by individual and ejaculatory frequency. Thus, the more frequent the ejaculations, the smaller the volume. (³)
  • Semen volume is also influenced by age, decreasing from an average of 2.8 mL in people aged 45 to 47.8 years to 1.95 mL in men older than 56 years. (³)
  • It is whitish with a gelatinous texture; its appearance may vary depending on lifestyle and general health. (²)
  • The presence of blood known as haemospermia, is an entity that causes anxiety in many men despite being a painless symptom. Most of the time benign and self-limited, it may be indicative of underlying pathologies. Various causes have been recognized: idiopathic (60%), congenital (ejaculatory duct cysts) (30%), inflammatory (25%), infectious (15%), obstructive (2.2%), traumatic, malignant (3 .2%), among others.   • Recurrent haemospermia requires special urological evaluations such as cystourethroscopy, seminal vesiculoscopy, magnetic resonance, in which anomalies or obstruction of the ejaculatory duct can be observed, in which it would be an indication to perform a transurethral incision in the opening of the duct for its resolution (¹⁴,¹⁵)
